HOW TO APPLY:

1. Complete ALL contact information
2. Tell us about yourself and experience (optional)
3. Submit Performance Video (REQUIRED - Dancer/Host):  Live, continuous flow, unedited performance video of 2-3 minutes in front of an audience showcasing transitions, tricks, and stamina on the pole
4. Practice videos are welcomed if you have not performed for an audience; the same criteria applies: continuous, unedited footage
5. You are encouraged to submit 2-3 videos for both options
6.  Individual files cannot exceed 10 MB.
